Once you have sent in your logo (in vector format) and filled out the form (https://dashride.com/app-setup/), then it typically takes 4 weeks for the app to be released. It takes some time for Apple to release the app on iTunes app store due to their intensive review process, so most of this waiting time is due to their processes.
